System of Care California Health and Human Services

The DxF is an opportunity to create equitable and affordable health information exchange, so that patients, providers, and other healthcare entities can access the health information they need to receive and provide timely, effective, whole person care. California Health and Safety code requires most providers and healthcare entities to sign the DSA by January 31, 2023. The agencies shall ensure that a process is developed for counties and partner agencies that are parties to the memorandum of understanding to request interdepartmental technical assistance from the joint interagency resolution team. The CARE (Community Assistance, Recovery & Empowerment) Act creates a new pathway to deliver mental health and substance use disorder services to the most severely impaired Californians who too often suffer in homelessness or incarceration without treatment. The CARE Act moves care and support upstream, providing the most vulnerable Californians with access to critical behavioral health services, housing and support.

The California Children and Youth System of Care State Team , recently reviewed submitted Memorandum Of Understandings to identify and highlight examples of exceptional System of Care practices. Identifying such practices may add value to other counties system of care practices and support partner agencies to further implement MOUs in counties across the state. Over the course of the last year, the state and county teams have made an impressive amount of progress toward implementation of AB 2083.

Medical Equipment Loans

Too often we are focused on delivering specific programs or services within our government silos that we do not take the time to look at the how our programs and services complement one another to best address the needs of the children and youth we are serving. We recognize that it is our obligation to ensure that the services we are providing are coordinated, timely, and trauma-informed. We must come together as one government to break down silos and build a culture that is focused on delivering services that are person-centered and not program-centered.

It became quickly evident, that there is a need for medical equipment in our community. Northern Management Services originally opened on February 1, 1991 by Bruce Fasel and Bob Passmore. NMS operated as an independent organization dedicated to helping persons with disabilities reach their full potential and maintain their independence. Watching individuals transition from an institutional setting to a community setting, provided NMS the unique opportunity to see people grow, develop, and become more independent. This experience highlighted the importance of being person-centered and the power of choice.

AB 2083 MOU Guidance

The legislation calls for the establishment of a Joint Interagency Resolution Team to provide guidance, support, and technical assistance to counties with regard to trauma-informed care to foster children and youth. More than ever, meeting the needs of our children and families requires our continued collaboration as interagency teams. As a resource to state, county, and local staff, we developed system profiles that provide an overview of the services offered by our various systems that all serve children and youth. Prior to receiving services, a thorough assessment is completed to assess the person’s needs. A plan of service is then developed with input from the individual, family, and health care providers. Monitoring of these services is ongoing to ensure that the plan is working and adjustments are made as needed.

Caprock Home Health Services, Inc. offers a wide variety of home health care services and personal care across Texas with offices located in Abilene, Amarillo, El Paso, Lamesa, Lubbock, Odessa, Plainview, Plano, San Angelo, San Antonio, Uvalde, and Wichita Falls. MOUs were selected based on a review of each of the required AB 2083 MOU components, for meeting, exceeding, or indicating an exemplary element in implementing the local Children and Youth System of Care. The three MOUs highlight practices that improve and promote collaboration and communication across systems to meet the needs of children, youth and families and can be a model for system partners. Overtime, we noticed a noticeable number of requests for various medical equipment, of which we would provide if in stock.

Community has skilled nurses and therapists who will enter the home to perform these tasks and more. CHHS via its partnership with the University of Maryland Francis King Carey School of Law offers nationally-recognized graduate-level academic programs to prepare the next generation of emergency management leaders. CHHS can provide a wide array of services to allow an individual’s needs to be met while remaining in their own home; regardless of their age or disability. Services are offered throughout Northern Michigan, including the Gaylord, Grayling, Cheboygan, Roscommon, and Houghton Lake areas.
